Long Term Care

Great CarePerry Green Valley Health Care is a 24-hour licensed long-term care facility.  We serve persons who may have a chronic illness or disability or may have conditions requiring daily medical attention that prevents them from living independently.

Our health care services include medical and non-medical care for individuals who need assistance with daily living activities such as dressing, bathing, grooming, toileting and eating.  Residents’ vital signs are regularly checked including temperature, blood pressure and pulse.  Our highly trained nursing staff will monitor the residents’ daily health needs, assess any changes that arise in physical and/or mental abilities, and work with residents to maintain good health.  Our staff adheres to rigorous guidelines for notifying a resident's physician(s) and family when certain conditions arise.

Medication Management

Medications are administered and recorded for each resident by certified personnel under the direction of the resident’s personal physician(s).  We are pleased to work with your local pharmacy of preference.  Some durable medical equipment and supplies are provided by the facility.  Please check with our Administrator for a complete list of items furnished by the facility.

Restorative Care

Our specially trained nursing assistants offer restorative care daily or as recommended by licensed therapists. Restorative care can help an individual to regain skills that may have been weakened or lost due to illness or injury, such as range of motion, walking, standing, personal grooming, etc.  Restorative care may also be offered when a resident is released from other forms of therapy.  Services may include physical, speech or occupational therapy sessions.  Designated and consistent staff members conduct the sessions and becomes familiar with the resident’s normal level of function, which results in better continuity of care, greater ability to identify problems, and prompt response to modify an existing treatment program to meet the resident’s needs.

Great LifePayment Options

Individuals who reside in Oklahoma nursing homes may apply for Medicaid assistance.  Oklahoma Department of Human Services provides guidelines to determine eligibility.  Local inquiries can be made to OKDHS Noble County Office at 205 15th Street in Perry.

Residents may also privately pay for all room, board, and fees.  For residents who satisfy the eligibility requirements of their long-term care insurance, the facility can assist with a resident’s insurance reimbursements by filing monthly claims to the insurance agency.  Medicare does not pay for long-term care residency if that is the only care needed.

I made a cold call visit to Green Valley.  No appointment, just walked into the facility and wanted to check it out, watch and observe, gather some information.  What I witnessed, discovered and experienced during that visit opened the door to many beautiful long-term relationships with an incredibly compassionate and passionate cadre of staff who truly care about caring for others.  Their commitment to service, to a cause greater than self, to expressions of joy and laughter as they hug and love on the residents makes my heart happy beyond measure.  The care they provided to mom in her final months and hours Is beyond description.  And they do it again and again and again for all residents.
